It is preferred that your school takes the time to complete this online form thoroughly, as this will help the team to prepare for the consultation. We suggest printing this form before you submit it should you require it for your records.

About our service:
The role of Down Syndrome Queensland’s education consultants providing SDSS services in schools is to work with the student’s support team to identify barriers to learning, with the goal of improving access to, and participation in, curriculum and other aspects of school life, in order to improve outcomes of eligible school-aged students with an intellectual disability. We aim to deliver specialist education services that are complementary to those offered by the education sectors and are not a duplication of services that are already available in schools. This process involves consulting and collaborating with school support teams to identify individual goals and learning targets, and to develop specific strategies and resources to facilitate learning engagement, participation, and learning. We provide advice to schools on adjustments and strategies that contribute to improvement in individualised learning, which can be implemented by teachers and school staff to enhance educator capability, improve outcomes for students with intellectual disability, and proactively engage and include the students in all aspects of schooling.

In order to provide the best available service to schools, we monitor student outcomes and school satisfaction ratings to identify areas for improved outcomes. Data collection is embedded throughout our consultation process in order to track the efficacy of our services and improve outcomes for all stakeholders.
